BILLING
The patient or guarantor will be responsible to pay all charges accumulated for your healthcare.
In an effort to maintain current records on your insurance and address/phone information, we require our patients to complete a sign-in sheet and provide your insurance cards at check-in for every appointment.
To assist our patients with their insurance coverage information, our office will verify eligibility and benefits with your insurance carrier. We will inform our patients of their estimated copayment expenses. Copayment expenses are due to be paid on the day of your appointment at check-in. Please be prepared to pay by cash, check or bank card/credit card.
Our office will file a claim with your insurance company. You will be billed any balance remaining after your insurance pays and your co-pay is applied (if applicable). The balance will be due upon receipt of your statement.
If you do not have insurance our office requires payment in full for medical services on the date of your appointment. You may be eligible for a nominal cash discount for payment at the time of service. If you are unable to pay in full, our office will discuss other options for payment, including payment arrangements and Financial Assistance.
If balances are not paid in full, your account will be forwarded to a collection agency and finance charges will begin to accrue.
Note: If your insurance carrier requires a referral, it will be your responsibility to obtain the referral from your primary care physician (PCP). If you do not have a referral for your appointment, your appointment will need to be rescheduled.
MISSED APPOINTMENTS
Due to the nature of our specialty and to assist with patient flow, we require 24 hour advanced notice of appointment cancellations.
- Excessive cancellations and no shows could lead to patient dismissal from our practice.
- If you are late to your appointment 10 minutes or more, your appointment may need to be rescheduled.
- As a courtesy, our office will attempt to confirm your appointment the day before your appointment.
PRESCRIPTION REFILLS
To assist your provider to provide you with your medication refill/s, please contact your pharmacy directly. You should allow 72 hours for any refill. Your pharmacy will contact our office if they have any questions related to your prescription.
NOTE: Our office does not process prescriptions refill requests after noon on Friday, on weekends, or holidays.
